Kindergarten Requirements All students attending school in the State of Indiana are required to have the following immunizations listed below, prior to the first day of school. |
Students entering Kindergarten should have a complete History and Physical, a vision screening, and a dental screening. This will help your child with a great start for their school days to come. Forms are available at the clinic of each school. |
DPT/DTaP 5 doses (4 doses are acceptable if the 4th dose was given on or after the child's 4th birthday) OPV/IPV 4 doses ( 3 doses are acceptable if the 3rd dose is on or after the child's 3rd birthday) MMR 2 doses HepB 3 doses Varicella 1 dose (Parental history of chicken pox is acceptable. Parent must write a note stating the month and year child had chicken pox. The note must also contain parent's signature). |
